What is the NCLEX?
The National Council of State Boards of Nursing (NCSBN) created the NCLEX test to assess whether nursing school graduates are prepared to practice nursing safely. The NCLEX is the pinnacle of nursing students’ education and the last barrier to becoming a registered nurse.
The NCLEX is a computerized adaptive test (CAT), which means that the examination is tailored to each test-skill taker’s level based on how they perform during the real-time exam. Evaluation of nursing competencies is more effective and efficient because of this CAT technology.
Both the NCLEX for Registered Nurses (NCLEX-RN) and the NCLEX for Practical Nurses (NCLEX-PN) are administered by the NCSBN.

- Types of Questions on the NCLEX
The format of the NCLEX’s questions is variable. This indicates that the questions are modified by the computer based on the test-taker’s performance. There are 74-145 questions on the NCLEX-RN. There are 15 pre-test questions in all; however, they do not affect test-takers’ final scores. There are between 85 and 205 questions on the NCLEX-PN, including 25 that are not scored. A five-hour time limit applies to both exams.
The exam includes questions in a variety of formats. These formats consist of:
Multiple choice
Ordered response
Fill in the blank.
Hot spot questions
The NCLEX has between 80 and 90 percent multiple-choice questions. The Next Generation 2023 NCLEX revised its question formats and introduced two new ones: matrix, or grid, and cloze (dropdown), for students who will take the NCLEX in 2023 or later.
- How to Score on the NCLEX
The NCLEX employs a pass-fail grading scheme rather than a percentage-based grade. The system, though, isn’t as simple as it might seem.
The NCLEX customizes each test-taker’s scores using Computerized Adaptive Testing (CAT). The computer determines the next question based on whether you correctly answered the preceding questions each time a question is asked. The computer selects questions that you have a 50% chance of solving correctly.
The computer will use one of the following three rules to determine whether you have passed or failed as you continue to answer questions:
The computer stops the test when it is 95% confident that you have passed or failed, according to the 95% Confidence Interval Rule.
Maximum Length Exam Rule: If you are about to pass, the computer will apply this rule and force you to answer all the questions. After you finish the test, the computer determines whether you passed or failed.
The Run-Out-of-Time Rule states that if there is a lack of time and the computer cannot determine with 95% certainty whether you passed or failed, one of the following will occur:
If you don’t respond to the required number of questions, the computer will fail you.
The computer will use the final ability estimate for the questions you answered to decide whether you passed or failed if you answered the minimum number of questions.
Remember that the length of the exam does not determine whether you will pass or fail.
